Detroit Integrated Care provides bridge care and short-term medical services for adults across Michigan through secure telehealth. Bridge care is designed to support individuals who are experiencing gaps in healthcare access and need temporary medical support while transitioning to ongoing primary or specialty care.
Our services are intended to address non-emergent, short-term needs and are provided as part of an integrated care model that prioritizes continuity, safety, and appropriate referrals.
Short-term medical services may include:
Temporary medication refills when clinically appropriate
Evaluation of acute, non-emergent symptoms (e.g., influenza)
Review of recent hospital, emergency department, or rehabilitation discharges
Care coordination and referrals to primary care or specialty services
Support during transitions from incarceration, rehabilitation, or shelter settings
Services are individualized based on clinical need and availability of follow-up care. Bridge care is not intended to replace ongoing primary care.
